Quebec ATT Instructions For NEW and EXISTING Members.
Procedure
- While logged in to the EOSC.ca website.
- Click on your name at the top right of your screen to access your member profile.
- You will see your member card. It will have your name and member number on it as well as the expiry date of your membership.
- Click on the “Printable PDF” option and open the card in Adobe (preferred) or another program you can print or download from.
- Print or download the PDF of your card and attach that with the other required documents to send to the QC CFO by your preferred method.
NOTE: THIS “PRINTABLE PDF” VERSION IS NOT AN ACCEPTABLE MEMBER CARD OR REPLACEMENT FOR A LOST CARD AT EOSC.
- Make a copy of your payment receipt to send to the Qc CFO.
- At the bottom of this page you will find a fill-able PDF version of the ATT application.
- Download that document and fill it in to be printed and mailed, or attached to an email as you prefer.
- NOTE: there is no postal code for the physical club address because it is not a mailing address.
The Qc CFO will need a copy of your member card (PDF download or printed) with the expiry date on it (from member profile).
A copy of your receipt (proof of payment).
